Vaccinations and Health Requirements
Providing a Safe and Healthy Environment for Our Guests
For the health and safety of your pet, all cats must have up-to-date records for all vaccinations, including FVRCP, FeLV (Feline Leukemia Virus), and Rabies. We do not accept home vaccinations or titer tests in lieu of vaccinations for any cat. All cats must also have had an exam within the past year if they are under 8 years old, or within the last 6 months if they are over 8 years old. If your cat is not current on his/her vaccinations or is due for a checkup, this will have to be scheduled with your veterinarian prior to your appointment. We prefer that vaccinations be administered 10 days before check-in. Additionally, all cats should be flea and tick free. All feline guests will be checked for fleas and ticks. If your cat shows signs of fleas and/or ticks they will be treated with advantage before check-in to ensure All Cats remains flea/tick free.

Boarding Agreement
The Boarding Agreement outlines the terms and conditions governing your cat's stay at All Cats Cat Lodge. Please read it carefully and sign where indicated. Include the name of a person (must be 18 years or older) who can act on your behalf in the event that we are unable to contact you during an emergency. This person may be asked to provide ID if he or she picks up your cat(s). If anyone other than you or your emergency contact comes to pick up your cat(s), we will not be able to release your cat(s) without first speaking with you. If time allows, please return this form to us in advance; otherwise, you may bring it with you to check-in. |

Medication Form
If your cat(s) takes any medications, vitamins, or supplements that will need to be administered during his or her stay, please complete and present the Medication Form. All medication that accompanies the pet must be in its original container clearly stating the type of medication and the dosage. Over-the-counter (OTC) medications can only be accepted with a pet when accompanied by a veterinarian's dosage recommendation. NOTE: There may be an additional charge for administering medication. |

Cat Assessment Form
The Cat Assessment gives us an opportunity to better understand the needs of your pet. You can describe the details of your cat's history, health, and behaviors and in turn we can customize our care and attention to your cat while boarding at the All Cats Cat Lodge. We require that the Cat Assessment is completed at your cat's first check-in and updated annually. You may print the Cat Assessment Form now, or fill one out in the All Cats Lobby. |

Authorization for Release of Vaccinations Form
If you need to request your pet's vaccination information from your vet, we can help. Download our Release Form and ask your veterinarian to complete it.
